Dental Screenings are coming up on January 13. Konza Prairie Community Health Center will provide FREE Dental Screenings to all students Kindergarten-12th Grade, unless Opted Out by a Parent or Guardian. See attached letter and (optional) Fluoride Consent Form. Letters and Consent Forms are being sent home with all K-6th grade students. 7th-12th graders can pick up a consent form in the high school office, if you would like your student to have Fluoride Treatment. Fluoride Consent forms are do back to school by January 11. To Opt Out, call your student's school and notify the Secretary or Nurse.
